| Basic Firefighter Advanced Firefighter Certifications Firefighters seeking these levels of certification must:
- Download the Basic and Advanced Firefigthter Program Form using the link below.
- Fill out the identification portion of the form. (Please print legibly or use a typewriter.)
- Successfully complete the required courses for the AFA certified Basic Firefighter level.
- Annotate the date of completion for each of these courses on the form. (Please print legibly or use a typewriter.)
- Send the completed form in to the Arkansas Fire Academy for verification (enclose $5.00 for your certificate and intial patch for this level). A copy of the form will be returned to you to track your progress for the next level. A copy will also be maintained at the Arkansas Fire Academy. Your certificate and patch for AFA certified Basic Firefighter will then be mailed to you.
- Successfully complete the required courses for the AFA certified Advanced Firefighter level.
- Annotate the date of completion for these courses on the form. (Please print legibly or use a typewriter.)
- Send the form in to the Arkansas Fire Academy for verification (enclose $5.00 for your certificate and initial patch for this level). The completed form will be maintained at the Arkansas Fire Academy. Your certificate and patch for AFA certified Advanced Firefighter will then be mailed to you.
Additional patches for a given level cost $3.50 each. Basic and Advanced Firefighter Certification Form Many of these courses can be applied to the Unified Fire and Emergency Response Associate of Applied Science degree program.
